How to Use This Template

  1. Initiate the form for any payroll-related change. The manager, HR, or the employee (for tax withholding and direct deposit updates) fills out Sections 1 through 4.
  2. Attach supporting documents. Tax form updates require the actual W-4 or state form. Direct deposit changes need a voided check. Garnishments need the court order. Do not process without documentation.
  3. Get approvals. Route through your approval chain. Compensation changes typically require owner or executive sign-off. Tax and deduction updates initiated by the employee may only need HR acknowledgment.
